Job summary

Baptist Health Specialty Pharmacy in Arkansas seeks an Ambulatory Clinical Pharmacist to support clinics within a multidisciplinary team. The pharmacist will ensure safe, efficacious, cost-effective therapy and will interact with patients and providers remotely and face-to-face as needed.

Responsibilities include medication reviews, disease management, transitions of care, motivational interviewing, and precepting students/residents. Day shift with flexible outpatient coverage.

Qualifications

• Graduate of ACPE-accredited College of Pharmacy with PharmD degree.
• Active Arkansas State Board of Pharmacy license.
• 3 years’ experience as a full-time pharmacist or completion of ASHP PGY1 residency.
• Strong verbal and written communication skills.

Responsibilities

• Provide medication therapy management in ambulatory settings.
• Collaborate with clinicians to optimize therapy and ensure safety and cost-effectiveness.
• Educate patients and providers; support transitions of care and adherence.
• Precept APPE students and PGY1 residents as defined by ASHP standards.

Summary:
Ambulatory clinical pharmacists will practice within the Baptist Health or embedded within a multidisciplinary clinical team to support clinics. The primary responsibility of this pharmacist is to assure the provision of safe, efficacious, cost-effective, and high-quality medication therapy disease management to patients in the ambulatory setting. The pharmacist in this role will interact with patients and providers remotely and face-to-face as needed. The pharmacist will consult with the patient and physician(s) to identify, resolve and prevent medication related problems, as well as establish mutually agreed upon treatment goals. The pharmacist assesses each of the patient’s medications to determine that each is appropriate for the patient, effective for the medical condition, safe given the comorbidities and other medications being taken, cost effective and able to be taken by the patient as intended. The pharmacist will titrate/manage/monitor the patient’s medication therapy to optimize clinical outcomes and will identify and address gaps in care. Additional responsibilities include patient and provider education, disease management, comprehensive medication reviews, transitions of care, motivational interviewing, precepting students and residents, and assisting in outpatient medication access and adherence. Pharmacists must have a patient-centered mindset, excellent problem solving and communication skills, who can thrive in an autonomous and evolving environment.

Required

• Graduate of an ACPE accredited College of Pharmacy with a Doctorate of Pharmacy degree
• Active license to practice Pharmacy from the Arkansas State Board of Pharmacy
• 3 years’ experience as a full-time pharmacist or completion of ASHP-accredited PGY1 residency program
• Strong verbal and written communication skills
• Eligible to precept APPE students and PGY1 pharmacy residents as defined by latest ASHP standards

Preferred

• Completion of an ASHP-accredited PGY1 residency
• Board Certification (BCPS or BCACP)
• Quality improvement experience

This job will be authorized80.00 hours bi-weekly.
