Qualifications

• Licensure: Active Georgia License Registered Nurse and current BLS.
• Educational requirements: Associates Degree.
• Minimum experience: Two (2) years as RN in inpatient/outpatient or one (1) year in Cardiology, ED or Critical Care; ANCC Ambulatory Nursing Certification.

Responsibilities

• Perform comprehensive physical and emotional assessments during in-person, phone and electronic encounters; develop plans of care in collaboration with providers.
• Triage walk-in, phone and electronic encounters; prioritize urgent care and follow physician orders.
• Administer and document medications; perform peripheral procedures and diagnostic support within RN scope.
• Recognize and report significant changes in patient condition; respond rapidly in emergencies per protocol.
• Provide patient education and support chronic disease management; participate in complex care coordination.
