Job summary

Sutherland Institute seeks an Ambulatory Clinic RN Care Coordinator to support physicians during clinic sessions, coordinate patient care, and educate patients to promote safety and outcomes.

You will manage patient flow, triage, and coordinate care across teams, documenting in EMR and ensuring understanding of the care plan by patients and families.

Requires BSN, 2+ years RN experience, current MO RN license, and BLS; full-time on-site position in Kansas City.

• 2+ years RN experience

Responsibilities

• Manage patient flow and triage in the clinic.
• Coordinate care between front desk, nursing, physicians, and clinic manager.
• Develop patient outcomes and coordinate with providers to meet goals.
• Provide nursing support to optimize patient outcomes and education programs.
• Educate patients and families about care plans and medications.
• Document assessments and plans in EMR.
• Maintain patient records and communicate results to physicians.
• Respond to patient treatment-related inquiries and referrals.
• Ensure patient confidentiality and compliance with standards.
