Job summary

Sentara Health Plans is seeking an on-site Registered Nurse in Chesapeake, VA to support a physician practice or ambulatory clinic. The role includes triage, patient education, immunizations, and care coordination with physicians and staff.

The RN will ensure safe clinical documentation in the EHR, participate in population health outreach, and contribute to a high-quality outpatient care experience. ADN is required; BSN preferred with 1–2 years ambulatory nursing experience.

Qualifications

• ADN required; BSN preferred.
• 1–2 years ambulatory/clinic or medical-surgical nursing preferred.
• Current state RN license and NCLEX-RN completion required.
• BLS certification required; AMB-BC preferred.

Responsibilities

• Assess patients and perform triage in person and by phone.
• Assist physicians with examinations and procedures.
• Administer medications, immunizations, and treatments.
• Provide patient education and self-management support.
• Coordinate referrals and follow-up.
