Job summary

Sentara Health in Hampton, VA seeks a Physician Practice and Clinic Registered Nurse to provide nursing care in ambulatory settings. The role assesses patients, supports examinations, administers medications and immunizations, and coordinates care with physicians and staff.

You will maintain clinical documentation in the EHR, support population health outreach, and help the practice run safely and efficiently. Required ADN with BSN preferred, RN license, BLS, and ambulatory experience.

Qualifications

• An ADN is required. A BSN is preferred.
• One to two years of ambulatory, clinic, or medical-surgical nursing experience is preferred.
• Current state RN license, NCLEX-RN, and BLS certification required; AMB-BC certification preferred.
• Skilled ambulatory assessment and triage; knowledge of chronic disease management, prevention, and immunizations.
• Strong patient education and care coordination; effective telephone triage; confident with EHR documentation.

Responsibilities

• Assesses patients and performs triage in person and by phone.
• Assists physicians with examinations and procedures.
• Adminsisters medications, immunizations, and treatments.
• Provides patient education and self-management support.
• Coordinates referrals, follow-up, and continuity of care.
• Manages patient messages and results within scope.
• Supports prevention and population health outreach.
• Maintains a safe, clean, and stocked clinical environment.
• Documents care in the electronic health record.
• Performs other related duties as assigned.
