Job summary

Damian Family Care Centers Inc. is seeking a Registered Nurse (RN) to deliver high-quality, patient-centered care in accordance with state and federal regulations.

The RN will coordinate, implement, and direct care with the interdisciplinary team to achieve optimal outcomes. Responsibilities include triage, assessments, medication administration, venipuncture, patient education, and referrals while maintaining accurate EHR records and HIPAA compliance.

Qualifications

• Current and valid New York State RN license and registration.
• Current Basic Life Support (BLS) certification from the American Heart Association.
• Current Infection Control certification.
• Minimum of one year RN experience, preferably in ambulatory care or FQHC.

Responsibilities

• Provide comprehensive nursing care using the nursing process (assessment, planning, implementation, evaluation).
• Perform triage, assessments, and data collection addressing diverse patient needs.
• Assist providers to ensure timely treatment within the care delivery system.
• Administer medications and immunizations as ordered.
• Perform venipuncture and collect lab specimens.
• Educate patients and families on diagnoses, treatment plans, and preventive care.
• Coordinate internal and external referrals for continuity of care.
• Communicate with pharmacies and insurers regarding treatment plans and approvals.
• Develop and implement nursing policies and procedures in line with standards.
• Maintain accurate EHR documentation and HIPAA compliance.
