Jefferson Health Ambulatory Clinics seeks a Registered Nurse to apply the nursing process while addressing the physical, psychological, emotional, and social needs of diverse patients. You will collaborate with ambulatory care teams to provide evidence-based nursing interventions and maintain patient-centered care.

Qualifications

• Specialized Diploma Graduate of an accredited school of nursing required
• Patient-and Family-Centered Practice: Knowledge of how to convey respect and collaborate with the patient and the designated care partner in care and care planning.
• Cultural Awareness: Knowledge of people with diverse backgrounds and the ability to convey respect and connect effectively.

Responsabilités

• Integrates evidence-based practice and clinical expertise to deliver care aligned with patient and family preferences.
• Collaborates with clinic care providers and supports timely communication between primary care and specialists.
• Documents patient interactions and nursing care in EMR per standards and regulations.
• Performs care coordination for abnormal results, triage, vaccinations, and other injections.
• Supports education for preventive care, chronic disease management, and medication management.
