Qualifications

• Specialized Diploma Graduate of an accredited school of nursing required.
• Knowledge of patient- and family-centered care and ability to collaborate with designated care partners.
• Cultural awareness and ability to connect with diverse patient populations.
• Effective communication skills and collaboration with the care team.
• Professionalism, confidentiality, HIPAA compliance, and ethical conduct.
• RN license in the state of Pennsylvania or state of practice.

Responsibilities

• Integrates evidence-based nursing into ambulatory care with patient and family preferences.
• Collaborates with clinic care providers to coordinate patient care and ensure timely communication.
• Supports implementation of clinical care pathways and participates in quality improvement.
• Delivers patient and family education on preventive care, chronic disease management, and medications.
• Documents all patient interactions and nursing care in the EMR per organizational standards.
