Job summary

Qualifications

• Graduate of an accredited School of Nursing; BSN preferred.
• RN license required with ambulatory care experience preferred.
• Basic Life Support (BLS) certification required.
• Proficiency in ambulatory care nursing and clinical knowledge.
• Strong communication and collaboration with care teams.

Responsibilities

• Administers medications and documents per SSH policies.
• Develops and updates patient plans of care with outcomes.
• Coordinates cross-continuum care and collaborates with teams.
• Ensures patient safety and quality through compliance and risk reduction.
• Communicates care plans to patients and families clearly.
