Aurora Medical Group (AMG) is part of Advocate Health, a large nonprofit health system with locations across Illinois and Wisconsin. This Referral Specialist role is based in Park Ridge, IL, with a Monday–Friday schedule and rotating 8:00am–4:30pm / 8:30am–5:00pm shifts.

You will support providers, connect with patients, and help ensure a seamless, compassionate experience daily as part of the ambulatory care team.

Qualifications

• AA degree or equivalent required.
• 3-4 years experience in a managed care organization, hospital or physician’s office.

Responsibilities

• Manage and navigate the centralized referral process.
• Coordinate discharge planning to provide cost effective care and quality outcomes.
• Participate in education and utilization management activities.
