Job summary

Advocate Health Care's Aurora Medical Group in Park Ridge is seeking a Referral Specialist to support providers, connect with patients, and ensure a seamless, compassionate experience.

You will manage the centralized referral process, monitor discharge planning for cost-effective, quality outcomes, and participate in education and utilization management within an ambulatory care setting.

Schedule: Mon-Fri, 8:00am–4:30pm or 8:30am–5:00pm, with a collaborative, patient-centered environment.

Qualifications

• AA Degree or equivalent is required.
• 3-4 years experience in a managed care organization, hospital or physician’s office.

What You’ll Do

As a Referral Specialist, you’ll play a vital role in the care team – supporting providers, connecting with patients, and helping ensure a seamless, compassionate experience every day.

As a key member of the ambulatory care team, you will:

• Management and navigation of the centralized referral process.
• Responsible for the coordination and monitoring of discharge planning to provide cost effective care and quality outcomes to members in accordance with their benefit coverage guidelines.
• Participate in education and utilization management activities
