Ascension's St. Joseph's Hospital in Milwaukee, WI, is seeking a Pharmacist to join our Pharmacy department on a full-time (36 hours) schedule, Monday through Friday 8:00 am–4:30 pm. This hospital-based role emphasizes clinical excellence, safety, and collaboration with care teams.

You will review complex medication orders, counsel patients, support interprofessional teams, and ensure safe dispensing across units, with a commitment to high-quality patient care.

Qualifications

• Pharmacist licensed by the Wisconsin Pharmacy Examining Board before hire.
• Doctor of Pharmacy degree required; Bachelor's or advanced degree acceptable as prerequisite.
• Minimum 2 years ambulatory care or chronic disease management experience.
• BCACP and 3-5 years ambulatory care experience preferred.

Responsibilities

• Assess, review, and optimize complex medication orders to resolve drug-related issues and advise interprofessional teams.
• Oversee compounding, dosing, and dispensing with safety standards across specialized patient populations.
• Direct consult with physicians, nurses, and care managers to streamline treatment plans.
• Counsel patients and educate on therapy regimens.
• Provide flexible coverage across pharmacy units as needed.

Doctor of Pharmacy

Bachelor's or advanced degree in Pharmacy

Minimum: 2 years ambulatory care/chronic disease management experience

Preferred: 3-5 years ambulatory care/chronic disease management experience BCACP
