Job summary

SSM Health in Wisconsin seeks a pharmacist to expand patient-care duties under collaborative practice agreements with Medical Staff. You will engage in medication management, education, and mentoring of students and residents.

The role emphasizes safety, outcomes, and teamwork across the care continuum, with responsibilities spanning counseling, therapy optimization, and quality improvement.

Qualifications

• Pharmacy degree from an accredited school (Pharm.D or B.Pharm).
• Two years’ experience or completion of PGY1 residency.
• Active pharmacist license in Wisconsin (IDFPR) or equivalent licensing requirements.

Responsibilities

• Promote patient safety by ensuring medications are indicated, effective, safe, and that the patient is adherent to therapy.
• Evaluate and optimize patient-specific drug therapy, assess effectiveness, and identify potential interactions or adverse effects.
• Assess patient medical problems, prioritize based on urgency, and design drug therapy plans.
• Provide detailed counseling on medications and disease states, assess adherence and costs.
• Consult with other health care professionals on drug-related treatment plans and outcomes.
• Identify best practices and participate in quality improvement initiatives.
• Precept pharmacy students and residents as applicable.
• Perform other duties as assigned.
