Job summary

University Health System in Texas seeks a pharmacist to deliver ambulatory pharmaceutical care in collaboration with pharmacy leadership, physicians, and an interdisciplinary team. The role focuses on education and patient care in the ambulatory setting.

The candidate will also serve as a primary preceptor for pharmacy students and residents, and implement P&T Committee decisions as written in the Hospital Formulary while complying with Health System policies.

Qualifications

• PharmD or BS in Pharmacy required.
• Texas pharmacist license active to practice.
• Completion of PGY1 residency or 3 years of applicable experience required.
• PGY2 residency in ambulatory care preferred.
• Board Certification in Pharmacotherapy or Ambulatory Care preferred.

Responsibilities

• Collaborates with pharmacy leadership, physicians and interdisciplinary team members to deliver pharmaceutical care in ambulatory setting.
• Serves as primary preceptor for pharmacy students and residents.
• Implements decisions set forth by the P&T Committee as written in the Hospital Formulary.
• Complies with Health System policies and procedures.
