The Ambulatory Care Clinical Pharmacist Specialist provides comprehensive pharmaceutical care to patients seen at hospitals and outpatient clinics through integrated clinical services, teaching, and research.

Qualifications & Experience

Two years of ambulatory care experience with disease state management. One year must be from a Pharmacy Practice Residency (PGY1) and one year from an Ambulatory Care Specialty Residency (PGY2) or equivalent experience providing patient care within a multidisciplinary team.

License & Certification

Licensed (or license‑eligible) as a Pharmacist in the state of Texas. Licensed (or eligible) Preceptor in Texas. Certification in Pharmacotherapy is preferred; if certified, maintain a minimum of 15 hours of continuing education annually.

Education & Training

Graduation from an accredited School of Pharmacy with a Pharm.D. preferred. Complete 15 hours of ACPE‑approved continuing education annually. Maintain competency in unique aspects of drug therapy and disease state management for ambulatory care patients.
