Job summary

JPS Health Network is seeking a clinical pharmacist with ambulatory care expertise to join its primary care clinics. The role focuses on medication management, collaborative practice agreements, and patient education within a busy Medical Home environment.

Candidates must hold a BCACP, complete PGY2 in Ambulatory Care, be licensed in Texas, and have at least four years of direct patient care experience including medication management. Daytime weekday hours are required.

Qualifications

• BCACP board certification required.
• PGY2 Ambulatory Care residency required.
• Texas State Board of Pharmacy license required.
• BLS certification required.
• Minimum 4 years of direct patient care with medication management.

Responsibilities

• Provides clinical leadership for pharmaceutical care in primary care clinic settings.
• Develops and optimizes pharmacotherapeutic regimens through collaborative practice agreements.
• Performs drug utilization reviews and monitors drug interactions and adverse events.
• Minimizes polypharmacy and creates patient medication action plans with self-management goals.
• Precepts students or residents and participates in education for staff and clinicians.
• Collaborates with multidisciplinary team to support patient care in the Medical Home.
