Job summary

Trinity Health MI in Grand Rapids, MI is seeking a part-time Clinical Pharmacist, Ambulatory Care to deliver patient-centered pharmacotherapy within the Trinity Health Medical Group.

The role includes disease management under collaborative practice agreements, diabetes, hypertension, anticoagulation care, and participation in education, quality improvement, and resident training across the health system.

Qualifications

• PharmD from ACPE-accredited school; PGY-1 residency required.
• PGY-2 Ambulatory Care Residency strongly preferred or commensurate ambulatory care experience.
• Licensed (pharmacist and controlled substances) in Michigan, or eligible.

Responsibilities

• Disease management and pharmacotherapy in ambulatory care.
• Collaborate on formulary and education programs; participate in quality improvement.
• Train and precept pharmacy residents and students.
• Provide education on pharmaceutical care to clinicians and patients.
• Support anticoagulation management service and other initiatives.
