Job summary

Trinity Health Saint Mary’s Grand Rapids is seeking a part-time Clinical Pharmacist, Ambulatory Care to provide drug therapy management for outpatient patients in the PCMH offices. This role supports disease management and collaborates with physicians to optimize therapy within approved clinical practice parameters.

Responsibilities include comprehensive medication reviews, daily informal consultations to improve pharmacotherapy safety and cost, and participation in the Anticoagulation

Qualifications

• PharmD from ACPE-accredited school.
• PGY-1 Residency completed.
• PGY-2 Ambulatory Care Residency preferred or commensurate ambulatory care experience.

Responsibilities

• Provide disease management within collaborative practice agreements (diabetes, hypertension, hyperlipidemia, asthma, COPD, anticoagulation).
• Conduct referral-based Comprehensive Medication Reviews with physician recommendations.
• Perform daily informal consultations to optimize pharmacotherapy safety and cost.
• Support Anticoagulation Management Service and other multidisciplinary initiatives.
• Participate in training of PGY-1/PGY-2 residents and PharmD students.
