• Collaborate with providers under Collaborative Practice Agreements for protocol-driven care.
• Lead chronic disease management and population health initiatives using data and registries.
• Support HIV prevention and treatment efforts, including PrEP/PEP programs.
• Ensure medication safety, conduct patient education, and document interventions in the EMR.
• Contribute to quality metrics (HEDIS/UDS) and program development.
