JPS Health Network is seeking a clinical pharmacist with ambulatory care expertise to join its primary care clinics. The role focuses on medication management, collaborative practice agreements, and patient education within a busy Medical Home environment.
Candidates must hold a BCACP, complete PGY2 in Ambulatory Care, be licensed in Texas, and have at least four years of direct patient care experience including medication management. Daytime weekday hours are required.
REQUIRED: BCACP board certification and PGY2 Ambulatory Care residency required RQEUIRED: 4 years of direct patient care that demonstrates medication management REQUIRED: Pharmacy license from Texas State Board of Pharmacy, BLS
o Shift: most JPS primary care clinics have extended daytime hours ie 7a-7p (4x10s is great as SEMH doesn't have space for the clinical pharmacist on Tuesdays. If the candidate can only work 8's but will give 40 total hours then we also need help at Poly clinic. )
o Days of the week: weekdays only
o Onsite seeing 16 patients per day as a mid-level provider
o Time split between 2 JPS primary care clinics
Provides clinical leadership for pharmaceutical care in area(s) of responsibility.
