Qualifications

Responsibilities

• Apply evidence‑based practice and clinical expertise to ambulatory care.
• Collaborate with care providers and staff to coordinate patient care.
• Support implementation of care pathways and quality improvement.
• Provide patient and family education on preventive care, chronic disease, and medications.
• Document care in EMR following organizational standards and regulations.
• Coordinate care, including triage and vaccination verification.
• Promote patient safety and continuous quality improvement.
• Engage in team huddles and accept feedback for practice changes.
