Jefferson Health Ambulatory Clinics seeks an experienced professional nurse dedicated to patient- and family-centered care. The nurse will use the nursing process to address physical, psychological, emotional, and social needs of diverse patients while collaborating with an ambulatory care clinical team to provide clinical nursing interventions.

Responsabilidades

• Integrates Evidence-Based Practice, clinical expertise, patients and family preferences for delivery of optimal healthcare.
• Collaborates with clinic care providers and supports communication between primary care and specialists.
• Implements clinical care pathways and participates in quality improvement opportunities.
• Documents all patient interactions and nursing care in EMR per organizational standards.
