Job summary

Tanner is seeking a Community Health Advocate - Ambulatory Care Management (Days) to help close care gaps for patients. You will use the EMR and payer portals to identify needs, schedule services, and submit documentation under supervision from the Quality and Training department.

Requirements include an Associate Degree or 2 years of college, at least three years of related experience, strong organization, attention to detail, excellent customer service, and proficiency with Word and Excel.

Qualifications

• Education: Associate Degree or 2 years of college coursework.
• Experience: Three years of related experience.
• Knowledge of medical terminology and EMR preferred.
• Must possess strong organization and time management skills.
• Must be detail oriented and able to work independently.
• Proficient use of Word and Excel.

Responsibilities

• Use EMR and payer portals to identify Care Gaps and schedule patients for services.
• Submit necessary documentation to close Care Gaps.
• Coordinate with data abstractors and Trainers.
• Work under supervision of department management and TMG leadership.
