Job summary

Valley Medical Center is seeking an Clinical Pharmacist, Outpatient - Primary/Ambulatory/Specialty Care, per diem, at VMC Main Campus in Renton, WA. You will provide pharmaceutical care, educate patients, and collaborate with clinicians to optimize therapy across ambulatory settings.

The role requires WA pharmacist licensure, PGY1 residency, and strong communication skills. Per diem hours and diverse specialty clinics offer flexible scheduling within the Specialty Pharmacy department.

Qualifications

• Licensed WA Pharmacist in good standing or eligible.
• PharmD degree strongly preferred for clinical practice.
• ASHP PGY1 residency required; PGY2 preferred in specialty areas.
• BLS for Healthcare Providers active or within 30 days of hire.
• Minimum 2–3 years clinical experience preferred.

Responsibilities

• Provide high quality ambulatory pharmaceutical care with documentation.
• Educate patients on medications and administration devices.
• Collaborate with providers to tailor drug therapies.
• Monitor and adjust therapy per CDTA and guidelines.
• Participate in disease state management and MTM activities.
• Lead and mentor residents, technicians, interns and students.
• Contribute to new clinical practice guidelines and quality plans.
• Assist in transitions of care and medication reconciliation.
