Qualifications

• Active Delaware Pharmacist License in good standing (or willingness to obtain)
• Board of Pharmacy Specialties (BPS) certification in Pharmacotherapy, Ambulatory Care, Cardiology, or Geriatrics (mandatory)
• Minimum of one year clinical pharmacy experience, including residency
• Experience with Collaborative Practice Agreements and ambulatory care pharmacy strongly preferred

Responsibilities

• Provide ambulatory clinical pharmacy care as part of CPAs
• Conduct medication management visits
• Collaborate with providers on evidence-based interventions
• Oversee specialty medication therapies
• Support patient access to medications via prior authorizations and financial assistance
• Precept pharmacy students and residents
• Contribute to research and quality improvement initiatives
• Document all interventions in the EHR
