Job summary

Live In Enid is seeking a registered nurse (RN) or licensed practical nurse (LPN) to lead a brand-new case management program. You will coordinate care across clinics and community resources, advocating for patients and guiding interdisciplinary teams to improve outcomes.

Qualifications

• Must be licensed as RN or LPN in Oklahoma.
• Ambulatory care experience preferred.
• Ability to lead in a team-based environment.
• Interest in interdisciplinary health teams and diverse populations.
• Excellent communication, organization, and time management.
• Familiarity with emerging professional and technical knowledge.
• Problem solving and multitasking skills.
• Compassionate with teamwork and advocacy abilities.

Responsibilities

• Coordinate care across clinic visits and community resources.
• Lead a collaborative, patient-centered care team.
• Track referrals and help patients access care.
• Provide basic health education and explain instructions.
• Support preventative care and affordable services access.
• Assist uninsured patients in identifying programs and applying for them.
• Help patients complete applications for patient assistance programs.
• Support informed decision-making about care and treatment options.
• Collaborate with medical team throughout the case management process.
• Engage with patients regularly to monitor progress and satisfaction.
• Document case information accurately and complete forms.
• Promote cost-effective care and high-quality outcomes.
• Assess motivational and psychosocial needs.
