Job summary

Advocate Masonic Hospital is seeking an experienced Advanced Practice Provider to join our Ambulatory Cardiology team. The role focuses on developing a comprehensive outpatient cardiology practice with a high-acuity program, including pre-operative assessments, post-hospital visits, and long-term ambulatory management.

You will collaborate with interventional and general cardiologists, providing education to patients and families, and supporting care coordination between inpatient and outpatient

Qualifications

• Demonstrated high level of clinical proficiency and strong decision-making skills.
• Excellent communication and collaboration with care teams.
• Proficiency with electronic medical records and care coordination.

Responsibilities

• Provide high-quality outpatient cardiology care, including pre-operative assessments and post-discharge follow-ups.
• Collaborate with interventional and general cardiology teams and support staff.
• Educate patients and families on diagnoses, procedures, medications and care plans.
• Participate in quality and safety initiatives and ongoing professional development.
• Maintain thorough documentation in the EMR and ensure continuity of care across settings.
