Job summary

Mayo Clinic in Minnesota seeks a Licensed Practical Nurse (LPN) to provide direct patient care and clinical coordination in the GIM- Ambulatory Breast Diagnostics Clinic. You will support access to care, prepare visits, and communicate with patients, providers, and the multidisciplinary team to ensure smooth workflows.

Responsibilities include reviewing health history, performing chart reviews, coordinating tests, and maintaining high-quality patient-centered care within a fast-paced ambulatory

Qualifications

• Graduate of practical nursing program or passed LPN boards.
• Current LPN license as required by state.
• BLS certification for Health Care Providers where applicable.
• Ambulatory care experience preferred.
• Ability to work daytime hours and adapt to workflow.

Responsibilities

• Conduct patient visits and collect vitals per protocol.
• Complete pre-visit chart reviews for readiness.
• Perform telephone encounters to coordinate care.
• Collaborate with providers and care teams to streamline patient flow.
• Serve as liaison among patients, providers, and staff.
• Coordinate outside records and diagnostic information.
• Support continuity of care and patient experience.
