Mayo Clinic in Minnesota is seeking an LPN to provide direct patient care and coordinate within the Ambulatory Breast Diagnostics Clinic. You will support timely access to care, ensure readiness for visits, and serve as a liaison among patients, providers, and the care team.

Qualifications

• Graduate of a practical nursing program or passed LPN boards.
• Current LPN license or eligible by state requirements.
• Experience in ambulatory care preferred; teamwork and scheduling flexibility.

Responsabilités

• Conduct patient visits with health history review and vital signs per protocol.
• Complete pre-visit chart reviews for readiness and coordination of care.
• Perform telephone encounters to assess needs and coordinate care.
• Collaborate with providers, RNs, desk ops staff, and interpreters to support patient flow.
• Coordinate outside records and diagnostic information for evaluation.
• Promote communication and continuity of care across the team.
• Practice within LPN scope and Mayo Clinic policies.
