Job summary

Mayo Clinic in Minnesota seeks a Licensed Practical Nurse (LPN) for the GIM Ambulatory Breast Diagnostics Clinic. The role provides direct patient care, clinical coordination, and acts as a liaison among patients, providers, and the care team.

Responsibilities include patient visits, chart reviews, telephone encounters, and supporting timely access to care in a fast-paced ambulatory setting. LPN licensure and BLS are required with preferred ambulatory experience.

Qualifications

• Graduate of a school of practical nursing or passed LPN Boards.
• One year LPN experience within the last five years preferred in ambulatory care.
• Current LPN license and BLS for Health Care Providers as required by region.

Responsibilities

• Conduct patient visits and review health history per protocols.
• Complete pre-visit chart reviews for readiness and coordination.
• Perform telephone encounters to assess needs and coordinate care.
• Collaborate with providers, RNs, desk staff, interpreters, and admin to support patient flow.
• Coordinate collection of outside medical records for evaluation.
• Promote communication across the multidisciplinary team.
