Job summary

Mayo Clinic in Minnesota is seeking a Licensed Practical Nurse (LPN) for the GIM Ambulatory Breast Diagnosis Clinic to deliver direct patient care and coordinate care in a fast-paced ambulatory setting.

You will perform health history reviews, medication reconciliation, vital signs, and chart reviews, while collaborating with physicians, RNs, and support staff to ensure timely access and high-quality care for patients.

Qualifications

• Graduate of a practical nursing program with an LPN license per state requirements.
• 1 year of LPN experience in ambulatory care preferred.
• Experience in team-based care and electronic medical records.

Responsibilities

• Provide direct patient care and clinical coordination in a high-volume ambulatory breast diagnostics clinic.
• Review health history, reconcile meds, and record vital signs during visits.
• Complete pre-visit/ pre-call chart reviews to ensure readiness and coordination of care.
• Conduct telephone encounters to assess needs and coordinate care with the care team.
• Collaborate with providers, nurses, schedulers, interpreters, and staff to optimize patient flow.
