Sweetwater Care in Petaluma, CA seeks a licensed vocational nurse to provide nursing care within the scope of practice, following physician orders and facility policies.

You will assist in medication administration, patient education, and rounds with physicians as needed, contributing to quality improvement and patient safety.

This full-time role requires an LVN license in CA and prior LTC experience; familiarity with PointClickCare is a plus.

Formación

• Active California LVN license required.
• Experience in long-term care preferred but not required.
• Knowledge of infection control and quality improvement programs.
• Competent with documenting and communicating orders and changes.

Responsabilidades

• Provide nursing care within scope of practice per standards and physician orders.
• Implement components of the Quality Improvement Program.
• Utilize Life Manual for patient placement and monitoring.
• Notify departments of patient changes (room changes, LOA, admit, discharge).
• Conduct patient and family education.
• Administer medications timely according to facility policy.
• Administer treatments per policy.
• Transcribe and carry out physician orders when permitted by state law.
• Participate in rounds with physicians as needed.

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.


• Understands, participates and assists in implementing the Quality Improvement Program.


• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.


• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.


• Conducts family and patient education.


• Demonstrates the ability to administer medications timely and according to facility policy.


• Demonstrates the ability to administer treatments timely and according to facility policy.


• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act


• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state of California


• One-year prior nursing management experience preferred, not required.
