Responsibilities

• Analyze reports and EMR data extraction to identify opportunities to improve outcomes based on medication usage and reduce pharmacy costs.
• Assess health plan members with predefined medical conditions to identify barriers to care and perform MTM services.
• Use evidence-based guidelines with the care team to achieve health goals of members.
• Collaborate with providers to implement clinical quality data initiatives for health plans.
• Maintain a 5-STAR rating of 90% or greater for members annually.
• Participate in committees, meetings, and audits as determined by the health plan.
• Develop and maintain policies and procedures that meet state and federal requirements.
• Inform providers and members of formulary changes through educational programs.
• Identify members' needs and intervene to prevent avoidable hospital admissions or readmissions.
• Document interventions to assess program success.
• Act as SME in pharmacy-related clinical measures for value-based care agreements.
• Maintain licensure and required BPS certification.

Astrya Global, a San Diego based medical staffing agency, is hiring a PHARM for a permanent assignment in Dover, Delaware.

Responsibilities for a PHARM:

• Analyze reports and EMR data extraction to identify opportunities that will improve outcomes based on medication usage (utilizing STAR and HEDIS) and reduce pharmacy costs to health plan members


• Assess health plan members with pre-determined medical conditions to identify barriers to care, perform self-management education, recommend screening, perform MTM services, and develop and implement personalized health improvement plans and goals


• Use evidence-based guidelines and health strategies in collaboration with the health care team to achieve overall health goals of members


• Collaborate with providers to implement clinical quality data initiatives as determined by participating health plans


• Maintain a 5-STAR rating of 90% or greater for members on an annual basis


• Participate in various committees, meetings, and audits as determined by the health plan


• Develop and maintain policies and procedures that support initiatives and meet State and Federal legal requirements and standards


• Inform providers and health plan members of the health plan's formulary changes on an annual basis through educational programs


• Identify health plan members' needs and intervene as appropriate to prevent avoidable hospital admissions or readmissions


• Document interventions to assess success of program


• Act as a subject matter expert in pharmacy-related clinical measures for value-based care agreements


• Maintain valid pharmacist licensure and required Board of Pharmacy Specialties (BPS) certification


• Perform all other duties as assigned, within the scope and range of job responsibilities
