Job summary

The Terraces of Phoenix, a HumanGood Life Plan Community, seeks a PRN Licensed Practical Nurse (LPN/LVN) to join our Skilled Nursing team and contribute to person-centered care for residents.

Requirements include an active LPN/LVN license in Arizona, CPR certification, and at least 6 months of Skilled Nursing experience. PCC experience preferred. PRN shifts include weekends and holidays with pay $30–$36/hour (DOE).

Qualifications

• Current LPN/LVN license in the state of practice.
• CPR certification.
• Minimum 6 months of experience in Skilled Nursing.
• Experience in post-acute, sub-acute, or senior care preferred.
• PointClickCare (PCC) software experience.

Responsibilities

• Provide attentive nursing care reflecting our Philosophy of Person-Directed Care.
• Coordinate assignments with care partners to ensure excellent resident care.
• Monitor resident needs and respond with compassion and professionalism.
• Support and hold team members accountable for quality care and resident satisfaction.
