Job summary

You will collaborate with the RN Case Manager, conduct physician-directed visits, and document observations accurately. The role includes patient education, team collaboration, and timely documentation to support quality home health care.

Qualifications

• Graduate of an accredited practical nursing program and licensed to practice in the respective state.
• Current CPR or BLS certification with hands-on demonstration required and maintained.
• Minimum of one year of nursing experience; home care or hospital nursing preferred.
• Excellent clinical skills and strong written and verbal communication abilities.

Responsibilities

• Deliver direct patient care per approved treatment plans and clinical protocols.
• Collaborate with the RN Case Manager and healthcare team to ensure effective patient care.
• Conduct patient visits per physician treatment plan, ensuring regulatory compliance.
• Document care provided and patient status accurately and timely.
• Provide patient education to patients and caregivers on care plans and health management.
• Demonstrate sensitivity to patients' needs, customs, and family concerns.
• Participate in case conferences and in-services or orientations.
• Complete and submit all required documentation promptly.
