Job summary

Eventus WholeHealth is seeking an Attending Physician to lead population health initiatives within our Outcomes-Centered Healthcare model at client facilities. You will work closely with Advanced Practice Clinicians to achieve excellence in patient care and professional client relations.

You may also serve in a Medical Directorship capacity, providing clinical leadership and oversight within one or more facilities, ensuring high-quality, accountable, team-based care.

Qualifications

• MD or DO required; Board cert in IM, FM, or Geriatrics preferred
• CMD certification a plus; if serving in Medical Directorship, required within 2 years
• Active unrestricted medical license; DEA registration as required
• 2–3 years clinical experience in post-acute, long-term care, or value-based care preferred
• Experience collaborating with interdisciplinary teams and facility leadership
• Familiarity with EMR systems (AthenaHealth preferred) and care coordination workflow

Responsibilities

• Lead population health initiatives within Eventus WholeHealth’s OCH model
• Collaborate with Advanced Practice Clinicians on patient care and facility leadership
• Provide high-quality, team-based care across assigned facilities
• Possibly serve in designated Medical Directorship capacity for clinical leadership
