Lutheran Life Villages in Indiana is seeking a compassionate Social Worker to support residents’ psychosocial well-being and rights within our nursing facility. You will coordinate care plans and serve as the primary social services contact for your caseload.

Qualifications

• Bachelor’s degree required; social work license/certification preferred.
• Minimum of two years' experience in long-term care or related healthcare setting.
• Fluent English communication and interpersonal skills.

Responsibilities

• Coordinate, develop, and maintain individualized care plans for residents' psychosocial health.
• Serve as primary social services contact for assigned caseload, ensuring compliance with standards.
• Support residents adjusting to life in the facility and facilitate socialization.
• Identify and resolve resident and family concerns with leadership.
• Document social histories, assessments, and care plans; maintain progress notes.
• Coordinate relocation and discharge planning; arrange referrals to outside resources.
• Assist with precertification during skilled stays and communicate with insurers.
