Catholic Charities Twin Cities is seeking a Case Manager III in the Aging and Disability Services to support older adults and individuals with disabilities.

You will conduct intake and annual assessments, develop service plans, coordinate with partners, advocate for clients, and maintain thorough records. The role emphasizes trauma-informed, person-centered approaches, cultural awareness, and collaboration within a busy team.

Qualifications

• Bachelor’s or Master’s degree in social work or related field preferred.
• Experience as a social worker/case manager in a public or private social services agency.
• Ability to work with diverse and/or low-income populations, including homelessness, unstable housing, and/or mental illness.
• Must have a valid driver’s license and proof of current insurance if transporting clients.

Responsibilities

• Provides initial and annual assessments and case management services to a complex, often long-term group of clients, including in-home visits.
• Prepares, evaluates, and maintains a service plan to meet client needs and link to Catholic Charities programs and community resources.
• Advocates for clients and assists with required paperwork for services and benefits.
• Coordinates with supervisors and educates community systems to facilitate linkage, referral, crisis management and follow up.
• Maintains complete case documentation and records as required by funders or licensure.
• May transport clients per program guidelines and needs.
