Job summary

Northern Arizona Hospice, part of LHC Group and the Optum family, is seeking an Afterhours Registered Nurse to deliver comprehensive hospice care after regular hours, including nights and weekends. You will triage, evaluate eligibility, and perform afterhours visits with a focus on patient and family needs.

Salary range is $74,560 to $111,840 annually for full-time employment, with benefits and sign-on bonus.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• Current CPR Certification.
• 1+ years of clinical experience.
• Valid driver's license, reliable vehicle or access to public transportation.

Responsibilities

• Triage patient needs over the phone until an in-person visit by a nurse.
• Respond to crisis calls after hours with urgency for patients and families.
• Perform initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Conduct afterhours admissions, death, recertification and routine visits.
• Assess and reassess physical, psychosocial, emotional and environmental needs every 15 days.
• Document problems, goals, interventions and patient/family response.
