Job summary

Optum's Wythe Hospice is seeking an Afterhours Registered Nurse to deliver comprehensive hospice care after regular hours, including nights and weekends, in Virginia. This full-time salaried RN role requires a Virginia RN license, a valid driver's license, and reliable transportation.

Travel to Grayson, Carol, Wythe, Montgomery, and Pulaski counties is expected. Responsibilities include triaging needs by phone, evaluating eligibility for hospice within 48 hours, and performing after-hours visits

Qualifications

• Current unrestricted RN licensure in the state of practice.
• Valid driver's license and vehicle or reliable transportation.

Responsibilities

• Triage patient needs over the phone until an in-person visit by a nurse.
• Respond promptly to after-hours crisis calls from patients and families.
• Perform initial nursing evaluation within 48 hours of referral.
• Conduct after-hours visits including admissions, death visits, recertifications, and routine visits.
• Assess and reassess patient/family needs every 15 days.
• Document problems, goals, interventions and responses to hospice care.
• Function in diverse home environments regardless of client background.
