Job summary

Heart of Hospice, part of the Optum family, seeks an Afterhours Registered Nurse to deliver comprehensive hospice care after regular hours, including nights and weekends. The role emphasizes triage by phone, in-home visits, and urgent crisis response to support patients and families.

The position requires valid RN licensure in Louisiana, driving eligibility, CPR eligibility, and 1+ years of clinical experience. Salary ranges from $70,832 to $106,248 with full benefits.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• LA: a minimum of two years full-time RN experience; hospice RN experience may substitute for RN.
• Current CPR certification or ability to obtain within 90 days of hire.
• 1+ years of clinical experience.

Responsibilities

• Triage patient needs over the phone until an in-person visit by a nurse.
• Respond to crisis calls from patients and families after hours with urgency.
• Perform initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Conduct afterhours visits including admission, death, recertification, and routine visits.
• Identify and reassess patient/family needs every 15 days or as needed.
• Document problems, goals, interventions, and patient/family responses.
• Function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition.
