Spectrum Health Systems, Inc. in Weymouth, MA is seeking a Continuing Care Coordinator to support clients transitioning from Clinical Stabilization Services back into the community.

This part-time role offers 24 hours per week with flexible shifts and a pay rate of $22.00-$25.00 per hour. You will link clients with housing, employment, medical and social services, and coordinate initial appointments with community providers.

Qualifications

• Experience in substance abuse treatment or case management.
• Strong documentation and organizational skills.
• Proficient in basic computer use for entries and EHR.
• High school diploma or equivalent is required.

Responsabilités

• Coordinate continuing recovery services for clients after CSS program completion and provide aftercare counseling.
• Perform case management for housing, job searches, medical services, social services, education and other supports.
• Coordinate continuing care referrals by building relationships with community providers and scheduling initial appointments.
• Collaborate with Clinical staff to ensure smooth transition from residential to continuing care.
