Graham Healthcare Group seeks an After Hours Registered Nurse (RN) to provide hospice care in patients' homes around Arona, PA. The role involves after-hours calls, symptom management, and family support, with travel across the designated territory.

The position is full-time and staff-based, offering a defined 40-hour week, 8-hour shifts, and a comprehensive benefits package. A $20,000 total bonus potential is available, including a $10,000 first-year success bonus and a $10,000 sign-on.

Qualifications

• Active Registered Nurse license in the state you work.
• Two years of RN experience; Hospice experience preferred.
• Reliable driver’s license for travel to patient homes.

Responsibilities

• Deliver patient-centered hospice care in patients’ homes in a geometric after-hours schedule.
• Respond to after-hours needs with urgent assessments and symptom management.
• Support families during evenings, nights, weekends and crises.
• Coordinate care and maintain accurate EMR documentation.
• Work independently while collaborating with an interdisciplinary team.
• Travel to patient homes within the assigned territory.
