Job summary

Advocate Health is seeking a Hospice After-Hours/On-Call Registered Nurse to provide skilled nursing care for hospice patients and families during evenings, nights, weekends, and other after-hours periods. The role involves responding to calls, triage, and occasional home or facility visits within Floyd, Chattooga, Gordon, Polk, and Bartow Counties.

Qualifications

• Graduate of an accredited school of professional nursing.
• Current unrestricted RN license in state(s) of practice.
• Current driver’s license and reliable transportation for patient visits.
• Current BLS certification if required by policy.
• Clinical nursing experience with assessment and critical-thinking skills.

Responsibilities

• Respond promptly to after-hours calls from hospice patients, families, caregivers, facilities, and the healthcare team.
• Perform nursing assessment and triage to determine the appropriate clinical response.
• Make in-person visits when clinically indicated based on patient condition and care needs.
• Assess and manage hospice symptoms (pain, dyspnea, agitation, nausea, etc.).
• Provide education, reassurance, and emotional support to patients and caregivers.
• Coordinate with the hospice physician, NP, pharmacy, DME, and interdisciplinary team.
• Complete after-hours hospice admissions and necessary documentation.
• Document telephone encounters, assessments, visits, interventions, and changes in the plan of care in the EMR.
• Provide handoff to the daytime hospice team regarding changes and follow-ups.
