Graham Healthcare Group's AHN Healthcare@Home team in Larimer, PA is hiring an After Hours Registered Nurse (RN) Hospice to deliver compassionate end-of-life care in patients' homes. This full-time Staff position provides autonomy within an interdisciplinary team and serves in the North Huntingdon, PA area.

You will handle after-hours calls, manage symptoms, document in EMR, and travel to patient homes within the territory. Eligible for a lucrative bonus structure and comprehensive benefits.

Qualifications

• Active RN license in the state you work.
• Minimum two years RN experience with hospice care preferred.
• Must have valid driver's license and be able to drive ~20% of workday.
• Willingness to work after hours and weekends as needed.

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life.
• Respond to after-hours and on-call patient needs by performing urgent assessments, managing symptoms and pain, and providing compassionate end-of-life support in real time.
• Support and guide patients and families during evenings, nights, weekends, and crisis situations, ensuring they feel informed, reassured, and cared for throughout the hospice journey.
• Apply strong clinical judgment and critical thinking to coordinate care needs, respond to changing patient conditions, and maintain accurate, timely, and compliant EMR documentation.
• Work independently, but never alone.
• Travel to patient homes within assigned territory.

Skills
