Responsibilities

• Performs initial nursing assessment on new admissions
• Completes and coordinates the plan of care with the care team
• Regularly reports patient progress to the clinical manager and physician
• Develops and maintains patient care progress records
• Records pain/symptom management changes and outcomes
• Educates families on patient condition, needs and care
• Makes referrals to other hospice care disciplines as needed
• Performs on-call responsibilities and after-hours services
• Executes other duties as assigned
