Gentiva Health Services, Inc. seeks an After Hours Hospice RN to respond to patient needs during evenings, nights, weekends, and holidays, delivering compassionate care when it is needed most.

The role requires active MO RN license or eligibility, 1+ year nursing experience, CPR certification, and reliable transportation to patient homes or facilities. Duties include direct care, care coordination, and documentation in EMR with a focus on quality improvement.

Qualifications

• Active RN license in MO or eligible to obtain.
• 1+ years of nursing experience.
• CPR certification.
• Driver's license and reliable transportation.

Responsibilities

• Provide direct patient care after hours in patients' homes or facilities.
• Serve as liaison between patients, families, physicians and the hospice team.
• Respond promptly to urgent calls, visits and symptom management needs.
• Document visits and communications accurately in EMR.
• Participate in quality assessment and performance improvement programs.
• Work independently while coordinating with daytime care team.
