Job summary

Silverado Hospice in South Houston is hiring an RN On-Call to provide after-hours nursing support for hospice patients and families. You will conduct timely assessments, update plans of care, and coordinate services across care settings in a mission-driven environment.

Qualifications include an active RN license and at least 1 year of nursing experience (hospice experience preferred), a valid driver’s license, ability to lift 50 pounds, and willingness to work outside regular hours.

Qualifications

Must hold a valid RN license in the state.
Minimum 1 year of nursing experience.
Hospice experience preferred.
Valid driver’s license and reliable transportation.
Ability to lift 50 pounds.
Must clear a criminal background check, physical, and drug screening.
Flexibility to work outside regular hours.
Comfortable working with animals/pets.

Responsibilities

Perform comprehensive nursing evaluations and after-hours assessments for patients with changes in condition.
Assess for signs and symptoms of discomfort, including pain, and take prompt action.
Initiate and update plans of care; provide nursing follow-up and caregiver instruction.
Submit accurate documentation within 24 hours of visits.
Coordinate services across settings to ensure quality and appropriateness of care.
Follow approved care plans, modify as needed, and communicate with physicians when appropriate.
