Job summary

Silverado Hospice in South Houston is hiring an RN On-Call to provide after-hours nursing support for hospice patients and families. You will conduct timely assessments, update plans of care, and coordinate services across care settings in a mission-driven environment.

Qualifications include an active RN license and at least 1 year of nursing experience (hospice experience preferred), a valid driver’s license, ability to lift 50 pounds, and willingness to work outside regular hours.

Qualifications

• Must hold a valid RN license in the state.
• Minimum 1 year of nursing experience.
• Hospice experience preferred.
• Valid driver’s license and reliable transportation.
• Ability to lift 50 pounds.
• Must clear a criminal background check, physical, and drug screening.
• Flexibility to work outside regular hours.
• Comfortable working with animals/pets.

Responsibilities

• Perform comprehensive nursing evaluations and after-hours assessments for patients with changes in condition.
• Assess for signs and symptoms of discomfort, including pain, and take prompt action.
• Initiate and update plans of care; provide nursing follow-up and caregiver instruction.
• Submit accurate documentation within 24 hours of visits.
• Coordinate services across settings to ensure quality and appropriateness of care.
• Follow approved care plans, modify as needed, and communicate with physicians when appropriate.

What You’ll Be Doing

Anticipated pay range

$39 - $41 USD
