Job summary

The On-Call LVN at Suncrest Hospice will provide care to hospice patients in homes and facilities, supporting a dedicated care team. After-hours on-call coverage is required, seven days on/off in a rotating schedule, with travel across the service area when needed.

Candidates must hold an active LVN license with at least one year of licensed experience, be able to work independently, and collaborate closely with nurses, social workers, chaplains, volunteers, and CNA leadership.

Qualifications

• Current LVN licensure in state(s) of practice with at least one year of licensed experience.
• On-call experience preferred.
• Ability to collaborate with a multidisciplinary team.
• Able to work 1-1 with patients with strong care team support.
• Capable of independent work including phone, email and meetings.
• Strong time management to schedule visits and complete documentation.
• Willing to travel across the service area with reliable transportation.
• Comfort with hospice regulations and EMR systems.

Responsibilities

• Provide hospice care to assigned patient group in homes, facilities, or hospitals.
• Conduct after-hours on-call visits seven days on, seven nights off.
• Coordinate with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Document patient visits and maintain EMR records.
• Ensure compliance with hospice rules and regulations.
